The information you provide in this application form will be used to determine suitability for registration in accordance with the Day Care of Children (Jersey) Law, 2002. It will be held in accordance with the Data Protection (Jersey) Law 2005, and publicised in the Childcare Register, and to third parties seeking daycare provision. The information you provide may also be shared with regulatory authorities in other jurisdictions.

Providing false or misleading information in this application is an offence under the Day Care of Children (Jersey) Law, 2002, Article 12 (d).
